Completing the Cycle: Re-Audit of Rotherham Specific Inpatient Physical Health Management and Documentation Following a 2021 Trust-Wide Audit

Aims Rotherham Doncaster and South Humber (RDaSH) NHS Trust completed a Trust-wide audit in August 2021 to look at aspects of physical health management their inpatient units. Good results were achieved relation new admissions having initial medical examination within 24 hours consideration being given as whether the patient had capacity make decision agree or refuse such an examination. However, inadequate anything more than appearance, pulse blood pressure conducted with chaperone, opportunity state preferences sex chaperone. This cycle by re-auditing above criteria units order assess ongoing progress against targets following recommendations. Methods A dip sample five patients per ward (two Acute Adult, one Rehabilitation, PICU two Older Adult wards) was used. Patients who admitted between 1st July 30th September 2022 picked randomly electronic records studied. Results Nearly 90% received doctor admission. But, whilst these undertook that just general observation, pulse, only 14% it documented they chaperone present. In addition, not single person offered choice choose gender Just third decline documented. Perhaps unsurprisingly, wards performed better this criteria. Conclusion continue perform well terms conducting timely examinations. we identified there is clear lack documentation around documenting someone has consent what would prefer them.
